Diabetes Mellitus is a condition wherein the body has increased blood glucose levels. There are two types of diabetes mellitus. The type 1 diabetes mellitus starts during childhood, making it synonymous to juvenile diabetes. In this type of diabetes, there is an impairment in the production of insulin by the pancreas resulting in increased blood glucose levels in the body. This is often treated with insulin shots and a proper diet plan. There are times that the patient may outgrow this diabetes, and there are some whose diabetes persists into adulthood. The second type of diabetes is the type 2 diabetes or adult onset diabetes. This type of diabetes is an acquired condition, usually affecting adults with several risk factors like smoking, obesity, improper eating habits and a sedentary lifestyle. Gestational diabetes may be considered of this type. In this condition, there may be a sufficient amount of insulin circulating in the body but the cells are impaired from receiving glucose for its utilization.

There are times that you show no symptom of this condition and yet later find out that they have it. This is not unusual since like Type 2 diabetes, it develops slowly through the years of too much stress on the body. People have been diagnosed with diabetes almost ten years after they get it. The symptoms vary for each individual, but the two common signs that are very indicative of diabetes are increased urination or polyuria and frequent thirst or polydipsia. This is commonly seen in diabetic patients because the increased glucose in the body causes water to be removed from the cells to be able to reach homeostasis or a balance in electrolytes in the body. When the cells get dehydrated because of the outward flow of water, it triggers the brain to feel thirsty. And it leads to the frequency in the urge to drink. Drinking water and other beverages will in effect increase urination. And this cycle goes on until the increased glucose levels in the blood go down.

Another major symptom to complete the three P symptoms of diabetes mellitus is polyphagia or extreme hunger. Since the body is trying to balance its electrolytes and other fluids in the body, it triggers your hunger centers to make you feel that you want to eat. Unfortunately, the more you eat, especially carbohydrates, the more you encourage this condition to progress. And the feeling of hunger will not go away.

Having Diabetes mellitus can also feel like you are coming down with the flu. You tend to feel very weak with a feeling of malaise. Sometimes, you may feel a bit feverish and lose your appetite. This is because your body is not absorbing enough glucose in its cells. This is because the high blood glucose levels impair the ability of the cell to repair, leaving the wound open to infections and other complications. Once this symptom occurs, and the body still is not responding properly to the infection, gangrene might set in. This is a reason why diabetic patients have to be very careful to not have any wounds in the body. And if they get one, to take care of it until it heals. Women can also develop long term bladder and vaginal infections as symptoms, which can be a problem in treatment.

Diabetes insipidus is a condition wherein the kidneys are unable to inhibit water from being released, leading to an increase in urination and the feeling of extreme thirst. Unlike diabetes mellitus which has an increased blood glucose because of the lack of insulin, diabetes insipidus is a rare condition wherein the kidneys are not able to conserve water while filtering blood. This is probably due to vasopressin, an antidiuretic hormone. ADH is a hormone produced in the hypothalamic part of the brain and sent to the pituitary gland for storage and eventual release into the body. Central diabetes insipidus is caused by the lack of ADH while nephrogenic diabetes insipidus is because the kidneys cannot respond to ADH. Major symptoms include increased urination and excessive thirst. This excessive thirst allows patients to drink large amounts of water to replenish the water lost in the urine. This condition may have evolved from an inherited disorder wherein male children receive and cause the disease on the x chromosome from the mother to the female. Nephrogenic diabetes insipidus may be caused by the diseases of the kidney like polycystic kidney disease and the effects of various drugs. If the thirst mechanisms are working fine and fluids used are just enough, then there are no significant effect on the fluids of the body and salt balance. If there is a lack in fluids consumed, then dehydration and hypernatremia will set in the blood. Central diabetes insipidus is caused by a damage or tumor on the hypothalamus or pituitary gland.

Symptoms of diabetes insipidus include uncontrollable thirst, a yearning for ice water, increased urine volume of diluted urine reaching 2.5 liters a day in mild diabetes insipidus to 15 liters a day in severe conditions and if a lot of fluids were ingested. Nephrogenic Diabetes insipidus is due to the destruction or damage of the kidney tubules. They are the ones responsible for the absorption and release of water. This defect makes our kidneys unresponsive to ADH. The defect may be due to a gene disorder or chronic kidney disorder. Some medications like lithium, tetracycline, can cause nephrogenic diabetes insipidus. One out of four cases of nephrogenic diabetes insipidus. Gestational diabetes insipidus occurs only during pregnancy and because of the destruction of ADH by the placental enzymes.

Genetically speaking, nephrogenic diabetes insipidus that appears in children after birth is usually due to a genetic cause that permanently damages the ability of the kidney to concentrate urine. It is common in the male population although the women can also pass the gene to the children.

To screen for diabetes insipidus, the doctor will perform several tests to determine the signs and symptoms of diabetes insipidus and diagnose it. "WHAT IS GESTATIONAL DIABETES"

Definition

Gestational diabetes is a carbohydrate intolerance of variable severity that starts or is first recognized during pregnancy.

Causes

Gestational diabetes is usually diagnosed during the 24th to 28th weeks of pregnancy. In many cases, the blood glucose level returns to normal after delivery.

It is recommended that all pregnant women be screened for gestational diabetes during the 24th and 28th weeks of their pregnancy. The symptoms are usually mild and not life-threatening to the pregnant woman.

However, the increased maternal glucose (blood sugar) levels are associated with an increased rate of complications in the baby, including large size at birth, birth trauma, hypoglycemia (low blood sugar), and jaundice. Rarely, the fetus dies in the womb late in the pregnancy.

Maintaining control of blood sugar levels significantly reduces the risk to the baby. The risk factors for gestational diabetes are being older when pregnant, African or Hispanic ancestry, obesity, gestational diabetes in a previous pregnancy, having a previous baby weighing over 9 pounds, an unexplained death in a previous fetus or newborn, a congenital malformation (birth defect) in a previous child, and recurrent infections.

Treatment

The goals of treatment are to maintain blood glucose levels within normal limits during the duration of the pregnancy, and ensure the well-being of the fetus.

Close monitoring of the mother and the fetus should continue throughout the pregnancy. Self-monitoring of blood glucose levels allows the woman to participate in her care. Fetal monitoring to assess the fetal size and well-being may include ultrasound exams and non-stress tests.

A non-stress test is a very simple painless test for you and your baby. An electronic fetal monitor (a machine that hears and displays your baby’s heartbeat) is placed on your abdomen. When the baby moves, its heart rate normally increases 15 to 20 beats above its regular rate.

Your health care provider can look at the pattern of your baby’s heartbeat in relationship to its movements and determine whether the baby is doing well. Your health care provider will look for three accelerations of 15 beats per minute over the baby’s normal heart rate, occurring within a 20 minute period.

Dietary management provides adequate calories and nutrients required for pregnancy and to control blood glucose levels. Patients should receive nutritional counseling by a registered dietician.

If dietary management does not control blood glucose levels within the recommended range, insulin therapy should be initiated. Self-monitoring of blood glucose is required for effective treatment with insulin.

Outlook (Prognosis)

There is a slight increased risk of the fetus or newborn dying when the mother has gestational diabetes, but this risk is lowered with effective treatment and careful watching of the mother and fetus. High blood glucose levels often go back to normal after delivery. However, women with gestational diabetes should be watched closely after giving birth and at regular intervals to detect diabetes early.

Up to 40% of women with gestational diabetes develop full-blown diabetes within 5-10 years after delivery. The risk may be increased in obese women.

According to the team Hillier, the trend remained high after research on the improvement of maternal weight, maternal age, number of pregnancies, ethnicity, and infant weight at birth.

"Our results indicate that the 'trail motabolis' children to experience the future of obesity seen in one, or more, abnormal condition in blood glucose tolerance tests," said Hillier and colleagues conclude. "The risk can be changed by the treatment of diabetes during pregnancy."

Causes

Diabetes is caused by a problem in the way your body makes or uses insulin. Insulin is needed to move glucose (blood sugar) into cells, where it is used for energy.

WHAT IS TYPE 1 DIABETES

Definition

Type 1 diabetes is a chronic (lifelong) disease that occurs when the pancreas produces too little insulin to regulate blood sugar levels appropriately.

Causes

Diabetes is a lifelong disease for which there is not yet a cure. There are several forms of diabetes, including:

* Type 1 diabetes, often called juvenile or insulin-dependent diabetes
* Type 2 diabetes, often called adult or noninsulin-dependent diabetes
* Gestational diabetes, which occurs during pregnancy.

For all types of diabetes, the metabolism of carbohydrates (including sugars such as glucose), proteins, and fats is altered.

In type 1 diabetes, the beta cells of the pancreas produce little or no insulin, the hormone that allows glucose to enter body cells. Once glucose enters a cell, it is used as fuel.

Without adequate insulin, glucose builds up in the bloodstream instead of going into the cells. The body is unable to use this glucose for energy despite high levels in the bloodstream, leading to increased hunger.

In addition, the high levels of glucose in the blood causes the patient to urinate more, which in turn causes excessive thirst. Within 5 to 10 years after diagnosis, the insulin-producing beta cells of the pancreas are completely destroyed, and no more insulin is produced.

Type 1 diabetes can occur at any age, but it usually starts in people younger than 30. Symptoms are usually severe and occur rapidly.

The exact cause of type 1 diabetes is not known. Type 1 diabetes accounts for 3% of all new cases of diabetes each year. There is 1 new case per every 7,000 children per year. New cases are less common among adults older than 20.
